One application form, filled in block capital letters and signed in either blue or black ink only. The application form is not to be printed double sided and in case the applicant is a minor (less than 18 years old), the form must be signed by both parents.
One (1) photograph (3.5*4.5 cm), the photograph must be clear and in sharp focus and must have been taken no longer than six months ago. The photograph must have a white background and the applicant showing neutral expressions and not smiling, it must show a close-up of the face and shoulders such that the face constitutes 70-80% of the photograph.
Original employment certificate issued by the applicant's employer printed on company letterhead. The letter must be addressed to the Embassy of Nigeria, Visa Section. Please prepare the letter as per the following guidelines:

- State the applicant's employment duration, job title and current monthly salary.
